It is a platforming action game, and it is identical to its predecessor. The Red Ninja, who is tasked with recovering five Elemental Crystals that the adversary has scattered across several locales, is the player's character when the game first begins. Similar to the original game, the first four rounds can be played in any sequence, and after finishing each, the player takes control of a new ninja. Each ninja character has a special ability, weapon, and ninjutsu. Some of the ninjas have new skills from the last game.
The player must employ a particular ninja's skill, depending on the stage, to locate the crystal. As a result, if the player doesn't yet have the required character, some of the stages must be performed more than once. Once all four of the enemy's crystals have been collected, the player can enter the enemy's main base, where the Black Ninja is guarding the fifth crystal.

Each ninja has a life gauge that depletes each time one of them is wounded by an enemy's weapon. Finding a heart object will restore it. The player's life gauge is permanently increased by a power-up item that appears in every round (except the last one).
The ninja is knocked back without suffering harm when they touch an enemy but not a weapon. The player loses a life if a ninja uses up their full life gauge. The game can be restarted if the player doesn't exhaust all of their lives.
